Check item number 326 in the following:

This has a wire fairlead.  The cam cleat is attached to the top of this gadget.
the line is passed through the cleat from the fairlead side.  This way, the line 
under tension is prevented from being pulled out of the cam cleat.  The only
way to release the line is to pull up on the bitter end.
