Jason,

I don't drive a pick-up but since I didn't see another answer in my digest 
version I will tell you what I can.  Both Yakima (which I own) and Thule are 
good, but expensive.  A lot of pick-up drivers will put one bar on the cab and 
then either use a commercial or build a rack for the bed.  You can also get 
trailer hitch racks for the back end.

You could probably put a second bar on the back of the cab, but the distance 
between the two would not be ideal.

Yes, Yakima ans Thule items are costly.  A good idea is to visit roofrack
shops - they sell and install both used and new racks.  I've paid about 1/3
or 1/4 for a Thule rack + towers for my sedan, since sedan itself is far
from being new and fancy.  Then I've got a pair of Yakima saddles and a pair
of Hulley rollers for nealry 80% of price - kayaking stuff isn't as numerous
as car accessories and isn't depreciating fast.  Finally I've bought an
"extention pole" from Home Depot for $ 3 or 4 (steel rod 7/8" OD or so),-
the round-section one from Yakima costs $30 or more, and I even don't know
if rectangular ones from Thule exist.  The idea of the whole setup - rack,
saddles, rollers, and DIY extention pole - was picked at the Guillemot
Paddling Technique forum, which I recommend to visit.  The case of pickup
truck was also discussed a while ago and could be found in their archives.
Or ask them again :-)
