Telling where the camera is mounted is practically giving away the answer,
but I guess not everyone knows that besides the SM Camera Cradle, I also
have a helmet cam.

You were busy pressing the camera shutter with your right hand instead of
sweeping the paddle and you fell over because of a lack of support from the
right hand.

  Actually, you probably had it set on automatic shutter click or extracted
it from video.  So I'm not sure why your roll failed, but the view point
does seem to be very high off the water.  Was this the paddle that snapped
in two, sending the camera flying towards the sky?
