I've used the SPOT in a few adventure races, and we usually tape it with electrical tape to the PFD shoulder straps. That seems to work pretty well. I really don't think your head will significantly block its view of the sky. It is difficult to read the LEDs when it is taped to your shoulder, however, as well as to verify that you've pressed the correct buttons.
