I recently wrote "thank you for your bravery-type letters" to a rescuer and a 
potential rescuer.  The last one was presented posthumously to the deceased's 
mother.

Two little girls had fallen into a lake.  One guy goes in to try and rescue 
them, but he is not a strong enough swimmer and he drowned.  A second guy 
went in.  One of the little girls also died.
