I used to use an inflatable free diving vest and found it a minimal problem to thread the strap down through the spray skirt, but the attachment point was quite high on that vest. That is the major reason I haven't opted for another. The style is out of fashion and I haven't invested a lot of time to figure out how to do it to a standard PFD. It would be a lot more comfortable though; having a slightly loose fit vs. a tight thoracic fit to keep it on in the water.
