The free program "GPS Trackmaker" is excellent for this, allowing one to 
use map images  or Google Earth  to define waypoints and routes for 
upload to the GPS. 
It also allows you to incorporate images (favourite campsites, sunsets, 
portage entrances, etc) into the same file as your gps information.

OziExplorer (which is shareware, not freeware) also allows you to place
photo links on your maps.
